CICCC Update—24 January 2017 Meeting
Changes at Terminals Pty Ltd
At the January 2017 meeting, Gary O'Sullivan advised the Committee about recent changes to senior positions at Terminals Pty Ltd. The CEO, David BozinoskI, has resigned and has been replaced as Interim CEO by Stephen Gumley. Alan Figgis has been appointed as The National Engineering Manager (a new position) and new Operations Managers have been appointed at Botany and in South Australia.
At Coode Island, a Graduate Chemical Engineer, Samuel Franklyn, has been taken on in a casual position.
Incidents over the reporting period.
Of the incidents that occurred, two were of particular interest.
One concerning the measurement of product levels in the tanks, is being addressed by the installation of dual radar measuring equipment. The measurements will be done independently, and cross checked to a tolerance of 2mm. The level information will be sent electronically to the control system for the facility. Terminals confirmed that manual checks were still done for customers, and for some statutory requirements.
The second, concerned the diversion of gases from the combustor to the Carbon beds, due to a high oxygen level in the inert duct. This occurred on a Saturday when the temperature was in excess of 35 degrees, and the high oxygen level (>5 %) was triggered by a tank at plant C. The diverted gases went to the Carbon Beds, but the suction fan circuit breaker at the carbon beds tripped and did not allow the vapour to pass freely. This in turn caused a high vapour duct pressure trip, and all tanks vapour control valves closed to prevent cross contamination. Gary was called out, and was faced with a problem to find what had gone wrong. He was aware that if the problem was not solved quickly, pressure in the tanks might increase leading to releases of gases from the safety valves, and potential off-site odour complaints.
Gary safely started the second combustor, relieving the pressure and allowing all systems to return to normal.
What was learned from this incident was the need for a fault manual, where relatively rare problems (and their solutions) will be established to assist in the diagnosis of such events.
